Direction for use:
Dissolve content of vial in 25 ml Sterile Water for Injection IP. Gently swirl to mix content of vial. Do not shake vigorously to avoid foaming. To be adminis- tered via intravenous infusion using a total infusion time of 30 minutes. Do not co-infuse Dalbavancin with other medications or electrolytes.
Dose: As directed by the Physician.
Store below 25°C.
Keep medicine out of reach of children.

CAUTION: The injection should not be used if it contains visible particulate matter after reconstitution.

Each Vial contains Daptomycin sodium hydroxide 350 mg (Adjust to pH) IP Dosage: As directed by the physician. Store at 2°C to 8°C (in a refrigerator) Store in the original package. Avoid excessive heat. Read accompanying package insert for detailed dosage. reconstituted direction for use and precautions. Do not use the reconstituted solution if it contains any visible particulate matter.
